Shine Bright! Procurement Form

Thank you for donating an item or experience to Shine Bright! Your actual item, or a certificate for an experience, is due on September 14, 2021. Please fill out this form as accurately as possible. All items/experiences valued at $400 or more will receive a Free Dress day in September. The value of items donated should reach $250 per the Family Contract. If you are donating individual items, please fill out one form per item. If you are putting together a basket or package of items to be sold together, you are welcome to fill out one form.
